Even without being sandboxed, most malicious operations will require root permissions, which an app can't have without being granted explicit access from the user, and this new layer of security provided by the App Store makes it so even trivial operations such as 'reading from the user's Pictures folder' need to be explicitly declared. So even if I wanted to do sneaky stuff, I wouldn't be able to, and IMO you're given a much better guarantee of that than with source code (who guarantees the source match the app?).
